There is cartilage around the larynx in both men and women. At early ages there is not much difference. But when males hit puberty the voice box grows larger in proportion to females, causing an Adam's apple and a deeper voice.

In addition to this, the meeting point of the two portions of the cartilage generally forms an acute angle (of about 90°) in men, while in women it forms an open arc (of about 120°). This causes it to protrude more.

Women sometimes do develop a prominent Adam's apple, usually due to a hormonal imbalance. And some of these women, along with men who are self-conscious because of an unusually large Adam's Apple, sometimes opt for a procedure called Chondrolaryngoplasty, to "shave" the cartilage down to a more desirable size.
